DENTAL OPEN ENROLLMENT
OCTOBER 1 - OCTOBER 31, 2007
One of the benefits offered to members of EAA is a Dental/Vision Plan. EAA voluntary dues members have the choice of enrolling in one of the two plans EAA offers (in lieu of the City of LA Dental plans). In our effort to bring you the most service for your benefit dollar, we are offering you a dental benefit through Guardian (www.guardianlife.com). As an added bonus, if you are enrolled in one of the EAA dental options listed below, you are also enrolled in Superior Vision Plan and the Landmark Chiropractic / Acupuncture plan.
The Dual Option Program allows our members the choice between a PPO Dental Plan option (DentalGuard Preferred) and a DHMO Plan Option (Managed DentalGuard Care for CA). These costs of these plans are outlined below:

*The city will add $.09 to each employee’s bi-weekly deduction which is payable to the City as a collection fee.
This new plan will take effect January 1, 2008
Improvements over current plan:
IMPORTANT: Only Voluntary dues-paying members (Code 54) are eligible to participate in the EAA Dental/Vision Plan. You can check if you are eligible by looking at your pay stub and verify if Code 54 is next to the union dues deduction section. If you are not a voluntary dues-paying member you can become one by simply calling the EAA office and we will send a voluntary dues card with your Dental Information Packet. Members can only participate in one dental plan, either EAA's plan or the City's plan.
Enrollment Procedures
EAA Dental Plans
To enroll or make changes in the EAA Dental Plan employees must select the EAA Dental Plan in the City’s selection process during open enrollment. The plan and/or changes will not take effect until January 1, 2008. The following steps will assist you with the various selections offered open enrollment period:
If we do not receive a completed application you will automatically be deleted from the EAA Dental Plan.
